FORMS & INFO
For all patients:
Most insurance plans do require a physical therapy referral or prescription from a doctor. Your insurance company can tell you if they require a referral. If so, download our referral form to take to your provider. When you are ready to schedule, we will make every effort to get you scheduled to accommodate your busy life. Please have multiple times & dates you are available so we can get you scheduled quickly.
To schedule your appointment, please call 253-564-2220
Each insurance company has its own rules that apply for reimbursement. We try to keep abreast of the latest changes, and it is very helpful to us if our patients also understand and comply with these requirements. Please always bring your insurance card and ID with you to each visit, and notify us promptly of any changes.
Please keep in mind that many insurance plans require the patient to satisfy a deductible, co-insurance, and/or co-pay as part of coverage. For your convenience we accept Visa, Mastercard, Discover, American Express, personal checks and cash. Should payment at time of service present a hardship we will be happy to negotiate payment plans that can meet your budget.
As a benefit to you, we work to verify eligibility of benefits prior to your first visit so out of pocket expenses are known prior to treatment. We will also let you know if your particular insurance plan requires you to obtain a referral/ prescription and/or pre-authorization for treatment. If your injury is due to a motor vehicle accident (MVA), we will bill your Personal Injury Protection (PIP) coverage as a courtesy. However, you will continue to receive a monthly billing statement until your account is paid in full.
We also have a "Cash Pay Policy & Rates" for those who do not have insurance or would prefer to pay out of pocket.
If you have any questions regarding insurance or billing, please contact our Billing Manager Melaney, at 253-564-2220
